Here’s how it works:
1. Ask someone to help you replicate the photos below using a smartphone or digital camera.
2. Submit your photos using the form below.
3. Dr. O’Keeffe will review your photos and our treatment coordinator will contact you.

Step 1 – Take your photos in a seated position, in a well-lit room. For best results, have someone take the photos for you, as they are hard to take on your own. Check that the lens is nice and clean. Supported file types are jpg, jpeg, png, gif. Max. file size allowed 3MB.
